1. Comprehensive assessment

We begin with an in-home or telehealth assessment to understand your mobility, symptoms, routines, goals and support needs. Family members, carers or support coordinators can be involved when appropriate.

2. Personalised therapy plan

Your physiotherapist creates a plan targeted to your goals. This may include mobility training, strength work, balance practice or functional rehabilitation.

3. Ongoing sessions

Therapy is provided where it works best for you, including at home, in the community, at school, in your workplace or via telehealth. Sessions may include strengthening, task-specific training, balance exercises, mobility retraining and functional skill building.

4. Review and progression

Your progress is monitored regularly. The program is adjusted as needed, and your clinician communicates with your broader support team to ensure your care remains coordinated.
